On evaluation you note a deformity in that the distal interphalangeal (DIP) joint of the middle finger on his right hand is in a slightly flexed position and he is unable to extend the joint. You also note that he has point tenderness on the dorsal surface of the base of the distal phalanx directly over the site of the insertion of the extensor digitorum tendon.
