I’m not quite sure what answer you’re looking for, but syncope (passing out) is usually caused by a drop in blood pressure, but irregular heartbeats and other cardiac issues can cause loss of consciousness as well. You mentioned he is skinny, and hypoglycemia, low blood sugar that could be associated with not eating enough, can cause a person to pass out.
